To maintain strict hygienic protocols and a disease-free boarding environment, WE ask that you agree to the following TERMS AND CONDITION.

● For the safe and comfortable environment of our customer’s dogs and staff, please note that Puppy Square reserves the right to refuse entry of any aggressive and/or excessive barking dogs.

● All dogs must have completed up-to-date vaccinations on the premises. Dogs need to have a C5 vaccination. This provides immunity against distemper, hepatitis, parvovirus, parainfluenza and bordetella bronchiseptica (which cause canine cough). All vaccinations are recommended to given 5 days or greater before check-in because disease protection does not begin until five days after vaccination.

● All dogs must be de-sexed after 7 months.

● All dogs must have regular flea and worming details, and heartworm prevention details.

● All day care and boarding dogs must complete a behavioral assessment on their first visit.

● You acknowledge and understand that dog day care and boarding centers have inherent risks including the risk of illnesses or diseases being transferred between dogs.

● You acknowledge and understand that injuries to dogs may occur because dogs are allowed to play and interact directly with each other. You agree that Puppy Square will not be held liable for any injury or illness and expressly waive any and all claims against Puppy Square or its employees for any loss, injury or illness to your dog or their property while in the care of Puppy Square.

● You acknowledge and understand that Puppy Square may need to use corrective techniques with your dog, but at no time will this include any physical punishment of any kind, to maintain a controlled safe environment.

● When your dog requires immediate veterinary attention, you consent to Puppy Square transporting your pet to either the nearest vet or your preferred vet nominated upon registration. You understand and agree that you will be responsible for all medical and veterinarian costs incurred.

● Under no circumstances will Puppy Square, or its employees be held responsible for any disease or parasite (including Paralysis Tick, or Canine Influenza), injury, death, loss or damage of any kind whatsoever that may occur to any animal. You fully release and indemnify Puppy Square and its employees from all such liability including all costs, loss, damage and expenses (including all veterinary, medical and/or legal costs) incurred as a result of or in connection with your dog being in the care and control of Puppy Square.
